This is the original DVD Cover for the DVD ISO of Tsukuyomi Moon Phase released some time ago by Atarashii Fansubs (Polish fansubbing group).
I did not scan this but downloaded it off a bittorent: TokyoTosho. It is over 80MB bmp file in .rar file. So I have just cut out the
front cover and changed the bmp into jpeg. Have fun!
