I found the image of Sasuke online (it's one of those popular ones you can find versions of everywhere) and edited the eyes to have his new Mangekyo Sharingan. The seaside beach background I found online as well. I messed with the glow, brightness/contrast, and softness a bit to make it look more like the sun's out and shining. I wanted to make it more of a simple background and similar to the anime/manga scene in which he's at the seaside grieving Itachi and then revealing his new eyes. And yes, making the Mangekyo that small while still able to get some detail was a pain.

I tried to stick with the colors Sasuke is wearing for the background (which is a blend of two or three images) so it'd mesh well - backgrounds are my bane. He had black eyes so I added that new Mangekyo Sharingan of his. It is one of my favorites.
