I believe the two characters: Akiyoshi and Kanon are from a short story called Ginyuu Meika found at the end of the second volume of the Full Moon wo Sagashite manga.

Wonderful scan. I'm so going to use those flowers in a wallpaper soon. The couple is so cute. I guess I will have to try the Full moon series. It looks rather enjoyable :). Thanks for the lovely scan.
