Scanned it from my Artbook.
It's the same person. As a boy (Kei Yoshikawa) and as a girl (Megumi).
Megumi is Mikoto's girlfriend in Princess Princess. So Mikoto is IMHO gay. XD

    Kei is genetically female, hence using the feminine reading of his name (Megumi). So no, Mikoto is straight. Nice scan, though!
