Great scan.
Manatsu Kuroki (left) and Ichika Tachibana (right) from Uta~Kata. Ichika is wearing a white version of her "Mirror" costume from episode twelve (it was black in the anime). Costume designed by none other than Keiji Gotoh, who also directed the series.
The scan's original source is apparently Megami Magazine, Volume 61 (June 2005).
