Her name is Yuki Terai and she's created by Ken-ichi Kutsugi of Frog Entertainment.
I scanned it from the 'Virtual Beauties 2020' book.
Sorry for the low-quality, but it was the best I could do from the original print. If resized to a usable wallpaper size the visual quality will increase.
