it's from "W - Wish", a 12 or 13 ep long series that aired in Japan last season.
It's not a very good series, seeing as each ep was 15 min long and the plot was quite complex. But yea - that's where this scan is from.
Not sure where its from other than the text already on there. Found on moeboard, not my scan.
