I don't know why you considered that I just found it but I must say this picture was scaned by myself .if you need something to make sure I will give you.and then I'll never go here.

About the Author's name, there can be some excuses, e.g. not everyone know the romji for the kanji, just like most americans(and others) can't read kanji.
